Admissions Policy

Vivo Healthcare Orange Park accepts admissions 24 hours a day, seven days a week. 

Our admissions office will work closely with your care provider to make your admission process is a smooth and pleasant experience. Our Director of Admissions can assist you with insurance and Medicare/Medicaid guidelines, and help you in maximizing your benefits. We will be glad to discuss any questions or issues may have.

Facility tours are available at your convenience. To arrange for a tour, or for admission information, please call our admissions office at 904.264.3912, or complete and submit the admissions form.
